Management

During the procedure

Extravasation due to guide wire perforation noted at the time of injury can almost always be managed conservatively. This may be aided by low-pressure tamponade with an angioplasty balloon for up to 5 minutes. Extravasation during angioplasty is more treacherous as this implies a significant laceration to the artery that requires urgent and definitive treatment. Low-pressure inflation of an angioplasty balloon above the injury, ideally from the contralateral side, will allow proximal endoclamp control. With concurrent fluid resuscitation, the anatomy can then be reviewed to determine whether a stent graft can cover the injury or if surgical repair is required (Figure 2).
